Off to College Guides

Moving from home to college can be overwhelming for students with diabetes, as well as their families. CDN has compiled booklets with information and tips for students and parents to help make that transition a little easier. Request your copies of the Off to College Guides - we have versions for caregivers and one for students.

Off to Work Guide

The Off to Work Guide is the first of its kind for T1Ds transitioning to the workplace. It features helpful information and resources to help T1Ds thrive in a work environment by knowing how to advocate for their needs and knowing their rights, as well as advice from T1Ds who have been there.

Newly Diagnosed

“You’ve Got This: a Guide for Young Adults Newly Diagnosed with Type One Diabetes”  provides valuable resources and a peer-support model utilizing other newly diagnosed young adults’ stories to address the gaps faced by this population. The guide includes tips, tricks, and resources for adjusting to life with diabetes as a young adult in college and beyond.

Transitioning from Pediatric to Adult Care

Access information on the process of finding a new provider for your young adult patients from experts and young adults who have been there

Host an Off to College Event

Many clinics and organizations host "Going Off to College" or "Transitioning" events, where they bring high school students with diabetes and their families together to share information about this busy time of life. We want to make hosting these events even easier! Request your free guide and materials.

Insurance 101

Young adult patients often have a lot of questions as they transition to a new healthcare plan. We compiled the resources they need to make the best choice to manage their diabetes.

Quote Description

"If you’re really serious about getting people to better outcomes, it’s not just about medicine. It’s about understanding the people who use our medicines. You can’t do that sitting at a desk, wondering what people think. You need to get out to meet people and talk to them. They need to share some of their experiences."

"CDN is a terrific organization. It is wonderful to see new chapters being established across the country. Having peer groups where common issues can be discussed have been shown to be very valuable, especially as young adults move from pediatrics to adult clinics."
